motor-2x-05 The insured must submit the following documents:
  1. Travel Claim Form.
  2. Itinerary, Air Ticket, and/or Boarding Pass etc. that shown the departure and return dates.
  3. The policy schedule.
  4. Copy of passport.
  5. Copy of bank book for transfer the claim settlement.
  6. Any other documents the Company may require if necessary.
  1. Personal Accident, Loss of Life, Dismemberment, Loss of Sight or Total Permanent Disability. 1.1 Medical certificate.
    1.2 Death certificate.
    1.3 Copy of autopsy report and police daily report.
    1.4 Copy of beneficiary’s identification card and house registration.
    1.5 Any other documents the Company may require if necessary.
  2. Medical Expense * Please send the original medical report, invoices and receipts to Head Office by EMS only.
    2.1 Original medical certificate.
    2.2 Original receipts and statement.
    2.3 Any other documents the Company may require if necessary.
  3. Travel Delay.
    3.1 Written confirmation from airlines or carriers for the reason of delay including new for old flight.
    3.2 Compensation confirmation from airline/carrier.
    3.3 Any other documents the Company may require if necessary.
  4. Flight Misconnection.
    4.1 Written confirmation from airlines or carriers for the reason of delay including new for old flight.
    4.2 Airlines or carriers confirmation of the amount of refund.
    4.3 Receipts of Accommodation, meal and beverage.
    4.4 Any other documents the Company may require if necessary.
  5. Baggage Delay.
    5.1 Written confirmation from airlines or carriers for Baggage Delay with date and time.
    5.2 Delivery report for baggage with date and time.
    5.3 Receipts for essential clothing purchased.
    5.4 Airlines or carriers confirmation of the amount of refund.
    5.5 Any other documents the Company may require if necessary.
  6. Loss or Damage of Baggage / Personal Effects / Personal Money and Travel Cheque.
    6.1 Police report.
    6.2 Property Irregularity report issued by airlines, carrier, hotel manager stated detail of loss or damage and their expense.
    6.3 Receipts for the loss or damage property.
    6.4 Any other documents the Company may require if necessary.
