Personal & Health Claims Guidelines
In order to obtain a speedy and smooth service following an accident, please follow the guidelines set out below.

Hospital services within contract:-

1. AXA Healthcare  card or AXA Hospicare card must be used with any official card issued by a government agency with your photo
2. If there are any questions, please contact the service provider in the AXA Healthcare card or AXA Hospicare Card

Hospital services outside contract:-

Pleaes complete the claim form and provided the below document and then send to Claims Department (Health & PA), AXA Insurance PCL.

1. Health & Accident Claims Form
2. Original Receipt
3. Doctor Certificate
4. Copy of Insurance Card
5. Copy of ID. Card of the insured
6. In case of the insured as a child. Please attach a copy of the parents ID. card (father or mother).
7. If the insured want to transfer to bank account. Please attach a copy of book bank with certified the document.
