In the event of a claim arising, the following procedures should be complied with :-

1. Notify Claims Department immediately

             1.2 Non-office hours (After 1700hrs) and holiday, please contact - AXA Hotline 0-2206-5488 or
                    0-2642-6688 (24hours)

        2. In case of large claim or complicate claim. The insurer will appoint a loss adjuster to investigate the
             loss circumstance.

        3. In all cases, the information will be required with a completed claim form agreed by policy holder :-

•  Policy Number and Insured Name
•  Damage Property/ Circumstance
•  Proof of Ownership of The Damaged Property
•  Location of Loss
•  Date/ Time of Loss
•  Name and Contact Number of Witness (es)
•  Initial Loss Estimate
•  Contact Person and Telephone Number.

       4. Photograph of the damages should be taken before anything is disturbed.

       5. The Insured should provide AXA Insurance Public Company Limited with all documents related to
           such loss as follows :-

• Original or Copy of Invoices/ Receipts for Repairs/ Replacement.
• Police Report (if necessary)
• Third Party’s Details e.g., name, address, their insurance company, etc.A Copy of Receipt for Repairs/ Replacement duly signed by the third party (in the event of the damage was done by third party)

       6. Urgent action to mitigate further damage or make safe the area/ person should be proceeded
           immediately.

       7. Any summons obtained has to immediately notify to AXA Insurance Public Company Limited.
